All eight disciplines step by step, with hints on what belongs in each. At the end you print the report, save it as PDF or copy it as text.

D1Form the teamWho works on the solution? Names, department, role. One person leads.
D2Describe the problemPrecise enough that someone without prior knowledge recognises the defect: what, where, when, how many. Facts, not guesses.
D3Contain the problemWhat protects the customer until the cause is fixed? Blocking, sorting, inspecting, with date and result.
D4Find the root causesTwo questions: why did the defect occur, and why was it not detected? The five whys lead from the effect to the actual cause.
D5Plan corrective actionsWhich actions remove the causes from D4? And how do you prove they work before they are introduced?
D6Implement corrective actionsImplement, prove the effect, then withdraw the containment actions from D3.
D7Prevent recurrenceWhat do you change so the same defect does not return on other parts, lines or sites? FMEA, inspection plan, work instruction.
D8Close and recognise the teamClose the report, inform the customer, thank the team.

What belongs in the eight disciplines

An 8D report answers three questions for the customer: Are they protected? Has the cause been found? Will the defect come back? The eight disciplines order the work so these questions are answered in turn.

  • D1 to D3: protect. A team, a precise description with numbers, containment such as blocking and sorting.
  • D4: understand. Two causes, not one: why the defect occurred and why nobody detected it. The five whys lead from the effect to the cause.
  • D5 and D6: fix. Actions against both causes, with proof that they work. Only then does containment stop.
  • D7 and D8: learn. Update FMEA, inspection plan and work instruction, transfer to similar parts, close.

When it is not just one complaint

An 8D report settles one case. Whether the same cause sits in ten other complaints only shows when all cases are side by side. With Onagero your quality team builds an analysis of all complaints from your own data, with patterns by part, line and supplier.

Questions about the 8D report

What is an 8D report?

A report that works through a customer complaint in eight steps: from the team through containment and root causes to actions that prevent recurrence. It shows the customer that the defect is understood and fixed.

Who asks for an 8D report?

Mostly customers in the automotive industry, usually through their own supplier requirements. Many specify their own forms and deadlines; this template contains the usual content.

Where does the 8D method come from?

From Ford's manual "Team Oriented Problem Solving", first published in 1987. In Germany the VDA describes it in its volume "8D-Problemlösung in 8 Disziplinen" from 2018.

What is the difference between containment and corrective action?

Containment (D3) protects the customer while the cause still exists, for example by sorting. Corrective action (D5, D6) removes the cause itself; after that the containment can stop.

Is my report saved?
